La Peregrine Pharmaceuticals, Inc. , sta testando un nuovo farmaco per l'HCv il Bavituximab sperimentato in pazienti coinfetti HIV - HCV.
Il farmaco viene somministrato in un braccio con ribavirina contro ribavirina + interferone nell'altro braccio.
Si spera di avere buoni risultati visto che l'interferone non e' molto ben tollerato dai pazienti.
Chiaramente si punta all'eradicazione del virus HCV.

Peregrine Pharmaceuticals, Inc. (NASDAQ: PPHM), a clinical-stage biopharmaceutical company developing first-in-class monoclonal antibodies for the treatment of cancer and viral infections, today announced the completion of enrollment in the company's Phase Ib dose escalation safety study of bavituximab in patients coinfected with chronic hepatitis C virus (HCV) and HIV. Previously this month, Peregrine initiated a randomized Phase II HCV trial to evaluate 12 weeks of therapy with bavituximab, a phosphatidylserine (PS)-targeting monoclonal antibody with immune-modulating potential, in combination with the antiviral drug ribavirin versus standard of care, pegylated interferon alpha 2a and ribavirin.


"Completion of enrollment in our third Phase I HCV trial is an important milestone for our bavituximab antiviral program, and sets the stage for reporting clinical data at a medical conference in the second quarter of this year while we begin to evaluate combination treatment with the antiviral agent ribavirin in a recently initiated study," said Steven W. King, president and chief executive officer of Peregrine. "Though standard treatment for chronic HCV may soon evolve with the introduction of new targeted antiviral drug candidates, immune stimulation with interferon remains a critical component of therapy. Preclinical data support the potential combination of bavituximab and ribavirin and we look forward to seeing how this combination initially compares to standard interferon and ribavirin treatment for 12 weeks in our Phase II study for patients infected with HCV."


In prior HCV clinical trials, bavituximab administered as monotherapy in single and multiple doses demonstrated a positive safety profile with no dose-limiting toxicities or serious adverse events. Bavituximab as a monotherapy also showed promising on therapy antiviral activity of up to 1.5 log viral load reduction.


Bavituximab may address a fundamental "immune evasion" mechanism exploited by many infectious pathogens. A growing body of published data from researchers worldwide shows that bavituximab's PS target, exposed on the surface of cells infected by viruses and protozoan parasites, suppresses the immune system's ability to fight disease. PS-targeting antibodies such as bavituximab bind to PS and block the immunosuppressive signals created by the target, thereby allowing the immune system to mount a robust immune response against the pathogen.


About the Phase Ib HCV Trial

Peregrine's open-label, dose escalation safety study is designed to assess the safety and of bavituximab in up to 24 patients chronically infected with HCV and HIV. Patient cohorts received ascending dose levels of bavituximab weekly for up to 8 weeks. Primary endpoints include safety and pharmacokinetics, and secondary endpoints will measure HCV and HIV RNA by PCR. Peregrine Presents Phase Ib HCV/HIV Data on Single-Agent Bavituximab at EASL

Bavituximab Safe and Well Tolerated, Ongoing Phase II Trial Evaluating Bavituximab With Ribavirin

TUSTIN, CA, and BERLIN, GERMANY--(Marketwire - April 4, 2011) - Peregrine Pharmaceuticals, Inc. (NASDAQ: PPHM), a clinical-stage biopharmaceutical company developing first-in-class monoclonal antibodies for the treatment of cancer and viral infections, today announced data from its Phase Ib dose escalation safety study of bavituximab in patients coinfected with chronic hepatitis C virus (HCV) and HIV. In a poster presented at the 46th Annual Meeting of the European Association for the Study of the Liver (EASL), data show bavituximab administered as a single agent for 8 weeks was generally safe and well tolerated at all four dose levels.

"Bavituximab used as a single agent has demonstrated a consistent, acceptable safety profile in three Phase I HCV trials to date, and we have seen enhanced antiviral activity when using bavituximab in combination with the antiviral agent ribavirin in several preclinical viral disease models," said Joseph S. Shan, vice president of clinical and regulatory affairs at Peregrine Pharmaceuticals. "Our recently initiated randomized Phase II trial will assess early virology response of genotype 1 HCV patients after 12 weeks of therapy combining bavituximab with ribavirin as a potential alternative to the current interferon-based regimen."

Peregrine's Phase Ib safety study included 27 patients (85% genotype 1 HCV) coinfected with HCV and HIV. Coinfected patients typically have higher HCV viral loads and more rapid progression compared to patients infected with HCV alone. Patient cohorts received ascending dose levels of bavituximab weekly (0.3 mg/kg, 1.0 mg/kg, 3 mg/kg and 6 mg/kg) for up to 8 weeks. Adverse events (AEs) were considered mild or moderate and were consistent with other bavituximab trials. Three serious adverse events were reported, two of which were drug-related (rash and hypersensitivity). Patients were not premedicated. No dose-limiting toxicities occurred and a maximum tolerated dose of bavituximab was not reached. Although not a primary endpoint of this safety study, HCV and HIV antiviral activity (≥0.5 log10 reduction) were observed in all treatment groups during bavituximab therapy.

In an ongoing randomized Phase II HCV trial, Peregrine is evaluating the 12-week early virologic response (EVR) rate of previously untreated genotype-1 HCV patients treated with Peregrine's bavituximab (0.3 mg/kg or 3 mg/kg) in combination with the antiviral drug ribavirin (1000 mg) versus current standard of care, pegylated interferon alpha-2a (180 µg) and ribavirin. "Peregrine Contact" il nome della società non è una gran garanzia!! Very Happy Very Happy

Scherzi a parte, sull'epatite C stanno davvero lavorando molto e ci sono farmaci promettenti in arrivo. Oltre a quello da voi riportato anche il Telaprevir e il Boceprevir sempre da abbinarsi a interferone e ribavirina.
Insieme al Bavituximab riescono ad arrivare a percentuali altissime di negativizzazione anche nei genotipi 1 e 4 che con la sola terapia di interferone e ribavirina restavano sempre i più difficili da trattare.

Purtroppo oggi, nei pazienti coinfetti -hiv e hcv- il problema più grave è spesso l'epatite C, essendo nella maggior parte dei casi infezioni molto datate e quindi facili a trasformarsi in cirrosi.
